Dosage is adjusted based on serum potassium concentrations: for levels below 3 mEq/L, give 2ml per 4.5kg three times daily; for levels 3-3.5 mEq/L, give 2ml per 4.5kg twice daily; for levels 3.5-4 mEq/L, give 0.5-1ml per 4.5kg twice daily.
