Doxycycline is primarily bacteriostatic. It inhibits bacterial protein synthesis by blocking transfer RNA binding to the messenger RNA ribosome complex. Its effects are time dependent and concentration enhanced, with AUC/MIC as the main pharmacodynamic parameter.
Resistance is mainly mediated by efflux pumps or ribosomal protection proteins. Cross resistance among tetracyclines is common but depends on the resistance mechanism. For example, a mutation in efflux pumps causing resistance to tetracycline may remain sensitive to doxycycline.
