It also acts against a limited range of Gram-negative bacteria whose outer cell wall contains lipopolysaccharides and proteins.
Resistance to beta-lactams occurs through three main mechanisms: beta-lactamase production, altered or modified penicillin-binding proteins (PBPs), and decreased outer membrane penetration. Beta-lactamase enzymes break the beta-lactam ring, inactivating the penicillin. Cross-resistance exists between amoxicillin and other penicillins, especially aminopenicillins. Use of extended-spectrum beta-lactams may select for multidrug-resistant bacterial phenotypes such as extended-spectrum beta-lactamase (ESBL) producers.
